New Patients

Eastgate Medical Centre welcomes new patients. Please arrive 10 minutes prior to your first appointment to complete the registration form. 
Your photo ID and valid Medicare Card are essential for the first time visit.   Please bring your medication list, if you are taking any regular medicine.
For the first time vist for children, please bring the immunisation record book (Purple Book) and carer's or parent's photo ID.

NARCOTICS (Opiates) ​​or SLEEPING PILLS (Benzodiazepines). 

Emergencies

  • In the event of an emergency call 000 for an ambulance. 
  • An afterhours locum service (PAMHS) can be accessed by calling 1300 000 362.

  • Medical reviews (eg. Diabetes review)  
  • Screening (eg. Cervical Cancer Screening)
  • Immunisation (eg. Routine Childhood Immunisations)

RESULTS OF TESTS 
Please make an appointment to discuss the results with your Doctor to ensure that you obtain correct information and advise for your healthcare. Please do not ask to discuss over the phone as refusal may offend you. 

PRESCRIPTION REQUEST 
Please make an appointment for repeat scripts if required.

MEDICAL CERTIFICATE & REFERRAL REQUEST
Please make an appointment for medical certificates or referrals. Please be advised that Doctors are unable to backdate the medical certificarte or provide over the phone. 

CONTACTING YOUR DOCTOR 
If you require emergency advice please inform the Receptionist so that your call may be triaged ppropriately. General calls will be responded between consults, at lunch break or after sessions on the same day. If your enquiry can not be responded same day you will be notified by the receptionist. General electronic queries can be sent through to our email reception@eastgatemedical.com.au and will be responded via telephone or email.You can request our written policy on eletronic communication.

Your feedback is appreciated for the continued improvement of our practice.  You are welcome to provide any feedback by completing the Feedback Form available at the reception desk.

Privacy
Eastgate Medical Centre is committed to protecting our patients’ privacy according to the Privacy Act.  The practice uses a computerised, paperless system.  Access to computer records containing your medical information is restricted to medical staff. All hard copies of patient documents disposed of in a secure manner, after scanned and stored in the patient medical record database.
